Or fill in this form and send it back to the Ozmates offices - We will send you all relevant paperwork for the requested Holiday to be completed. To secure a place on a holiday, a deposit is required along with application forms filled out by the primary support person. Deposit will be refunded if your needs can not be met. Bookings will not be accepted without a 24 hour contact phone number. Passengers needs vary and must be indicated clearly on forms.
- Passenger requiring travel from their home town to departure point can be arranged at the time of booking. We can help with meeting passengers in Melbourne providing support the night before and after a tour. This does cost extra and is not included in the tour fare.
- All money is due 60-120 days prior to departure date of holiday and will be noted on the invoice.
- Ozmates Travel will send final travel plans and contact numbers for holiday approximately 2 Weeks before departure.
- Items not included: Travel insurance, personal spending money, drinks, morning, and afternoon teas, transfer to/ from departure point unless specified.
- Please ask about our Travel Insurance options when speaking to one of our friendly Travel Consultants.
- Payments can be made by Cheque, Money Order or Layby.
Your fare includes
• All Travel Our land based tours are in luxury climate controlled air conditioned coaches designed for comfort. TV/DVD, stereo, MP3, sound system, ample leg room, and air ride suspension add to our Pssengers comfort. All vehicles are equipped with UHF Radios and telephones giving us immediate contact with emergency services anywhere in Australia.
• All Accommodation is Quality motel accommodation on twin/Triple share basis. In some circumstances Cabin accommodated holidays may have 3 to 4 to a cabin.
• All Entry Fees To attractions and rides listed as per the itinerary
• All Meals A Continental breakfast, picnic style lunch and evening meals.
• Support Crew (Carers) Our holidays are designed for mobile, socially independent adults that enjoy travel and participating in new activities. Our Support Crew offer support and encouragement when necessary, while respecting the needs of those passengers that prefer to be less adventurous. For people with higher support needs please contact our office for further options
Note:
- All applications are subject to acceptance by Ozmates Travel. All applications must be signed by the applicant or next of kin (Where Applicable).
- Tour may be cancelled at the discretion of Ozmates Travel if minimum number is not reached. The deposit will be returned to the applicant or transferred to another of your choice.
- Cancellation charges do apply if a passenger cancels their holiday. Notice of cancellation must be made in writing to Ozmates Travel.
Cancellation fees are:
- Australian Land Based: 100% for 0-30 days prior to travel, 50% for 31-60 days prior to travel, $100 for over 60 days prior to travel
- Train, Plane Tours: 100% for 0-60 days prior to travel, 50% for 61-90 days prior to travel, $100 for over 90 days prior to travel
- International Tours: 100% for 0-120 days prior to travel, 50% for 121-150 days prior to travel, $100 for over 150 days prior to travel
- Ozmates Travel reserves the right to modify the itinerary in any way necessary at their sole discretion and there shall arise no entitlement to refund.
- Prices quoted are subject to alteration without notice.
- It is essential that tour participants take out their own travel insurance for the tour. Application forms available from Ozmates Travel.
- Suitcases may be used, soft bags are preferred. Luggage must not exceed 15kg.
- If assistance is required with medication, enough medication for the duration of the tour plus three days spare medication in dosages to be handed to our staff.
- Ozmates Travel can assist with budgeting, provided that notes are in small denominations.

Tour prices are based on a staff ratio of 1:5. Higher support is available and must be discussed at the time of booking - “Higher support does cost extra”. Please refer to ‘Terms and Conditions’ page located in the Application Form for further details.
OVERNIGHT SUPPORT BEFORE AND AFTER TOURS
Overnight support is available. Ozmates staff will meet at city locations and the airport. Cost is $340 per person per night. Costs include: dinner, bed and breakfast, staff support and transport to departure point. Cost is based on meeting at 5.30 pm the day before travel until 8.30 am the next morning. Any time over these hours will be charged at $25 per hour.
